You've asked the crucial question! Most of the behaviorally based treatments for cluttering (and stuttering) require self-monitoring, and some clients find it either too unnatural or too taxing to talk in a regulated way. I often try to explain that it's a little like what the broadcast journalism majors might go through when they are learning to talk in their "news anchor voice"...it's a new style of speech, and with practice it can become easier to implement. This is an issue, though, for some clients. We had one client a few years ago who gained great benefit from the pausing strategy, yet he didn't like doing it and quit therapy. A few months later, he returned, saying he wanted to try it again, and from that point on, made significant progress. As for many clients, it never did become entirely automatic for him, though.
