Thanks for your question. I do find that in order to keep up their skills, clients need some sort of hierarchy for structured practice. A new found overall awareness of looking for listener reactions does go a long way. However, new habits require structured practice to become more automatic. I heard somewhere that to really make a change, we need to practice a new reaction/behavior in the real life context at least 30 times. So I think hierarchies are good, and use them with my clients.
